Salary range: SCHADS level 4 - From $43+ per hour + 11.5% Super + Salary packaging option

genU are looking for a dynamic, experienced Case Manager to join our growing and diverse organisation. If you are passionate about aged care and enjoy a challenge, this role could see your career skyrocket! As a Case Manager within the Home Care Packages division, you will work within a client directed framework with a focus of ensuring the wellbeing of older people who are in receipt of a Home Care Package.
You will be responsible for developing, reviewing and monitoring care plans with participants that meet their individual needs and identify their individual goals. You will also actively engage and collaborate with a range of internal and external services partners to ensure a holistic service is delivered.
This role is based in Victoria, Geelong and travel to other offices may be required. Please note that office days are Monday- Friday.
About You?
Our ideal Case Manager will demonstrate the following:

Understanding of the social, psychological and physical aspects of ageing and disability including the role of carers and a commitment to the principles and practice of client directed services

Experience in community care, particularly care/case management

Knowledge, skills and experience in the delivery of behaviour management strategies
-
Ability to be flexible, innovative and independent as you respond to the changing needs to care recipients and team members
You will need the following:

Relevant Tertiary qualification and/or extensive experience in a human services discipline

Current NDIS Worker Screening Check
-
Current NDIS Working With Children’s Check
-
Current NDIS Worker Orientation Module certificate
