genU is looking for a caring and motivated Case manager to join our Home Care Packages team and support clients to live well and stay independent in their own homes.
In this role, you’ll work closely with clients, their families and allied health providers to develop and review individual care plans that are tailored, practical and focused on each person’s goals. You’ll coordinate services, keep track of changing needs, and make sure supports are delivered in a way that’s consistent, compliant and within budget.
It’s a busy, people-focused role where no two days look the same. You’ll build strong relationships, problem-solve as needs change, and genuinely make a difference in the lives of the people you support.

What you’ll do
As a Case Manager in our Community, Residential & Pathways division, you’ll:
- Develop and implement comprehensive, consumer-directed care plans focused on goal outcomes, funding requirements, and measurable results.
- Collaborate with consumers, families, and support networks to ensure care plans reflect individual needs and preferences.
- Conduct regular care plan reviews, monitor progress, and adjust supports in response to changing client needs.
- Coordinate with allied health professionals and service providers while maintaining accurate records and compliance with Home Care Packages guidelines.
What you’ll bring
- Tertiary Qualification in a human services field – this may include Social Work, Welfare, Disability, Case Management, Registered Nurse or Allied Health
- Demonstrated experience in community care, particularly care/case management.
- Understanding of the social, psychological and physical aspects of ageing and disability including the role of carers and a commitment to the principles and practice of consumer directed services.
- Flexibility and innovation in responding to the changing needs.
- Knowledge, skill and experience in the delivery of behaviour management strategies.
(You’ll also need an NDIS Worker Screening Check and Orientation Module Certificate – but don’t worry, we’ll help guide you through this.)
Bonus points for
- Strong understanding of Commonwealth and State funding programs, including Aged Care Quality and Safety Standards.
- Excellent communication and interpersonal skills, with experience in interviewing and conducting client assessments.
- Ability to work independently and collaboratively in a fast-paced, multidisciplinary environment.
- Strong analytical, budgeting, and problem-solving skills with advanced computer literacy (MS Office and Carelink+ desirable).
