In this exciting new opportunity as an Individual Support Services Organiser, you will be the first point of contact for rostering and enquiries for clients and support workers for the provision of individualised services in and around Warrnambool. We presently work with a wide range of different clients with their own unique requirements, so if you have had experience as an administrator with any of the following funding streams: NDIS, Aged Care, Home Care, Department of Veterans Affairs and Transport Accident Commission packages, this role is for you.

 

Key responsibilities

 

As an Individual Support Services Organiser, you will be responsible for, but not limited to:

  • Assisting with developing support plans, risk assessments, and rostering.
  • Frequent interaction with clients, families, and support staff, with a specific client caseload and managing a high volume of calls daily.
  • Managing and overseeing a caseload of multiple funding streams, including NDIS, TAC, Home Care Packages, CHSP, and DVA, each requiring different services such as personal care, domestic support, community access, sleepovers, and respite care.
  • Providing performance feedback with guidance from the team leader to staff.

  • Are well-organised and can maintain and update client and staff documentation efficiently.
  • Have knowledge and skills in support plans and risk assessments.
  • Have experience supporting clients with diverse needs and behaviours.
  • Have experience/exposure in rostering procedures and carelink rostering systems.

Requirements:

 

  • Certificate III in Disability/Aged Services or experience in the field of human services or equivalent.
  • Current NDIS Worker Screening Clearance.
  • Current Working with Children’s Check.
  • NDIS Worker Orientation Module Certificate.
  • Current and Valid Australian Driver’s License.
  • First Aid and CPR – within 12 months.
