genU is looking for a compassionate and proactive Outreach Case Manager to join our Pathways team. In this rewarding community-based role, you'll support people experiencing homelessness, housing insecurity, and vulnerability to access the services, resources and support they need to improve their wellbeing and achieve their goals.

What you'll do
As an Outreach Case Manager in our Pathways team, you'll:
- Support people experiencing homelessness, at risk of homelessness, and vulnerable Victorians living in Supported Residential Services (SRS).
- Develop, implement and review person-centred care plans that reflect individual goals, needs and preferences.
- Build strong partnerships with housing providers, health and mental health services, My Aged Care, and community organisations to coordinate supports.
- Advocate for clients, monitor outcomes, maintain accurate records and help people navigate complex service systems.
What you'll bring
- Qualifications in Social Work, Welfare, Human Services, Psychology, Disability, Community Services or a related field (or equivalent industry experience).
- Demonstrated knowledge of homelessness, trauma-informed practice and supporting vulnerable people in the community.
- Strong communication, assessment and relationship-building skills with the ability to engage effectively with diverse stakeholders.
- Excellent organisational skills with the ability to manage competing priorities and maintain accurate client records.
- Current Driver's Licence, access to a reliable vehicle and comprehensive insurance.
You'll also need an NDIS Worker Screening Check and Orientation Module Certificate – but don't worry, we'll help guide you through this.
Bonus points for
- Experience working within community services, aged care, housing or case management programs.
- Knowledge of Carelink Plus, My Aged Care, funding programs or other community service systems.
