MatchWorks is looking for a highly organised and people-centred Post Placement Support / Ongoing Support Consultant to join our Browns Plains team!

What you’ll do
As a Post Placement Support / Ongoing Support Consultant, you will:
- Provide tailored support and structured follow-ups to clients who have gained employment
- Build and maintain strong relationships with job seekers, employers, and stakeholders
- Develop individualised employment pathway plans that promote stability and independence
- Identify early risks or challenges and implement interventions to maintain ongoing employment
- Monitor local job opportunities and work collaboratively with PPS/OGS teams to share leads and support outcomes
What you’ll bring
- Strong time management and organisational skills, including diary management, accurate case notes, and the ability to manage a high, fast-paced caseload
- Confident, professional communication skills (phone, face-to-face and written) with proven ability to build rapport and work with diverse client groups
- Relationship-building strengths with high-level negotiation skills, the ability to think on your feet, and balance empathy with accountability
- Practical problem-solving capability with resilience under pressure, plus the ability to work autonomously including onsite workplace visits
- Demonstrated ability to achieve KPIs, exceed performance targets, and utilise knowledge of local support services to assist clients
Additional requirement:
- A current Driver’s Licence
(You’ll also need an NDIS Worker Screening Check and Orientation Module Certificate – but don’t worry, we’ll help guide you through this.)
